I have read and understood the policies and procedures provided by Knowle Football Club and stored under 'Documents' on Team App. I am aware of the Safeguarding Policy and agree to abide by the Code of Conduct.

I am happy for Knowle Football Club to use my contact details for Club and football purposes. I understand that my private information won't be used by or sold to third parties without my consent.

I am happy for the Knowle Football Club sponsors to contact me with any offers available to Knowle Football Club members.

I have completed a hard copy Player Medical Form and returned this to my Team Manager / Coach for safe-keeping and use in the event of an emergency. I have made my Team Manager / Coach aware of all relevant medical, physical and learning needs to ensure I / my child can be appropriately looked after by the Club during football related activities.

I hereby give consent for me / my child to participate in football related activities, Club and team events and accept that it is my responsibility to ensure my child is safely transported to and from all pre-arranged pick up points. If I am ever delayed I will make every effort to contact the Club in order to make alternative arrangements.
