I have read and understood the policies and procedures provided by Knowle Football Club and stored under the 'DOCUMENTS' folder on Stack Team App. I am aware of the Safeguarding Policy, agree to abide by the Code of Conduct and have shared behavioural expectations with any child players in my care.

I am happy for Knowle Football Club to share my personal details and those of any child players within the club, the leagues, the FA and other associated agencies, in the interests of providing footballing-related activities and services. I understand that my personal information won't be sold or shared with outside third parties without my consent.

I am happy for the Knowle Football Club sponsors to occasionally contact me with any offers available to Knowle Football Club members.

I have completed a hard copy Player Medical Form (as per FA good practice guidelines / copies available on Stack Team App under ‘DOCUMENTS’ folder) and returned this to the manager of my team for safe-keeping and potential use in an emergency. I have made the manager or my team aware of all relevant medical, physical, learning and non-physical needs in order to ensure that I / my child player can be kept safe and well whilst participating in all football related activities.

Under the Equality Act 2010, a person qualifies as disabled if they have a physical or mental impairment that has a ‘substantial’ and ‘long-term’ negative effect on their ability to do normal daily activities. As a football club, we have aspirations to be as inclusive as possible in order to maximise access to and enjoyment of the game we love. Please flag any conditions or impairments relating to you and / or your child player(s) that you think might help us provide a better, more inclusive experience.

This might include physical disabilities or non-physical differences such as formally diagnosed or emerging signs of neurodiversity such as ADHD or Autism. We will always make reasonable adjustments where we can, but we don’t know what we don’t know, so please help us by starting an open dialogue. If you’re not comfortable sharing anything here or via the medical form then we would really encourage you to speak to the manager of your team, one of our other coaching staff or a Child Welfare Officer at the club. There is training and support available to our coaches in this area.

I hereby give consent for me / my child to participate in football-related activities with Knowle FC, including club and team events, and I accept that it is my responsibility to ensure my child is safely transported to and from all pre-arranged pick up points at the agreed times. If I am ever delayed, I will make every effort to contact the club in good time in order to make alternative arrangements.
